Information in the MENTAL STATUS and SUBSTANCE USE behavior domains is gathered through clinical interview by a trained clinician, with the inventories completed by the assessment clinician.
The remaining 7 Domains are addressed in a questionnaire format designed to facilitate client self-report and independent completion. These questionnaires identify adolescent-appropriate strengths & needs, use language and sentence structure designed for ease of understanding, and support narrative and open-ended response to insure comprehensive collection of information.
This self-report format also helps to insure that the “client’s voice” and language choice is well-reflected within the assessment, and provides insight into client readiness-for-change. The hand-written format allows for additional identification of written communication skills and challenges, as well as any comprehension difficulties. For those youth who are unable to complete the Adolescent Assessment Questionnaire independently, this tool easily serves as a guide for a comprehensive structured interview.
